## Narrative

An unintentional roof fall occurred on the 4G return in entry #8 and partially in entry #7.  The fall did not affect ventilation or passage of men or cause personal injury.  It had been bolted with 6 foot roof bolts and it was cleaned up and arched. It measured 75 foot x 18 foot x 6 foot.
