A fall of roof was discovered in the 1ST Southwest Parallel in the #4 Entry extending over to the #3 Entry at x-cut # 13. This fall was previously reported on E-Document #1066129. Per a discussion with a D10 Roof Control Specialist it was determined that (2) separate roof falls could not be reported on the same 7000-1.
