A fall of roof was discovered in the 1st South East Mains in the #4 Entry at x-cut # 14. The fall was approx 60ft Long x 19ft wide x 6ft high. This form is filed to abate citation #8499151. The fall was previously reported on form #1064550. Per a discussion with Roof Control Specialist it was determined that 2 separate falls could not be reported on the same 7000-1.
