A fall of roof was discovered in the 1st Southeast Mains at x-cut #12, between #6 and 7 Entry. This fall was previously reported on form #1064550. Per a discussion with a D10 Roof Control Sprecialist it was determined that 2 separate falls could not be reported on the same 7000-1. This form is filed to abate citation #8499151.
