Narrative

On 8/19/2010 was notified that Dr. has taken EE off work due to a strained left knee. EE was supplying roof bolter when a rock fell between the roof bolts raking him down his left side, pushing against rib, injuring his left knee. Rock was approximately 5' long by 6"" thick.
