Narrative

Employee was adjusting the packing on the #7 MLP on the 19 MTX line. After completing the task, the employee was exiting/dismounting the pump platform using the steps provided. Upon stepping off the last step, employee lost her footing and slipped backwards hitting the ground with her lower back.
