## Narrative

Employee was working on a piece of equipment and dropped a tool. He went under equipment and retrieved the tool. When he went to get up from floor he hit the back of his neck where the hard hat tightener is against equipment. He went down to his knees for a minute and then got up. He felt okay that day, but started feeling soreness in shoulder next day.
