Narrative

Employee alleges a shoulder/neck injury when he attempted to guide an anode as it slipped and was dropping to the floor; felt a slight pull between shoulder and neck. EE's first exam at a medical facility was on Aug. 25; released to return to work full duty. As a result of a follow-up exam on Sep. 1, doctor prescribed PT. This incident is pending further investigation.
