Narrative

Employee was installing roof bolts and as he was reaching to remove the drill steel, a rock (approximately 18"" x 14"" x 4"") fell from the mine roof and mashed his right index finger against the roof bolter drill pot, resulting in a laceration and crushing injury. His right index finger was amputated at the middle phalange at the hospital as a result of his injury.
