Narrative

The injured employee was walking from entry #4 to entry #6 to unlock a piece of equipment. When crossing entry # 5, inby the feeder tailshaft, the corner of the inby rib fell out striking the employee in the face. The coal rib measuring 2'x2'x2' pushed the employee to the ground. He had surgery on 9/10/2010 for a deviated septum, which took him off work.
