Narrative

Employee was using a hand held grinder when a piece of the grinding stone broke off. The projectile struck the face shield he was wearing and deflected into his left cheek, the resulting laceration required 8 stitches. The investigation revealed the grinding stone was properly rated for the grinder and all guards were in place.
