Narrative

Miner was in process of bolting, drilled for bolt, noticed loose area, left machine and steel in last hole drilled and barred down loose material. Loose came out of hanging and hit machine, then came down and hit employee on ""L"" thigh. Loose measured 12"" x 8"" x 4"". Bruise to ""L"" Thigh.
