Narrative

A roof fall was discovered at 6:05a on the B-2 Panel (#3 Heading in #5 Crosscut, 600' outby the loading point). The area was bolted with 6' SuperTwist bolts and 12' Point Tension Cable Bolts. No ventilation or equipment was disturbed. No one was injured as a result of this fall.
