## Narrative

On 6/13/2010 - Employee reported to medic on site that over the last 2 - 3 weeks he has had gradual onset of left elbow pain when he grips something or picks it up.  Employee does not have a specific injury or time.  There was no incident.  On 10/6/2010 he had surgery on his left elbow for tendonitis. 10/10/10 was his 1st shift missed.  Occ. Illness/LT
