Narrative

EE was trying to manually operate a pneumatically actuated valve. When EE applyed pressure to move the manual operating handle, the valve clutch slipped, causing his right middle index finger to get caught between the valve and a bolt. EE sustained a fracture to his right middle index finger. Restricted duty, no Lost Work Days.
