## Narrative

The employee was utilizing a rock hammer to tap on a slip-fit packing gland located on a shuttle car tram motor, when a piece of  metal broke off the hammer and embedded in the employees chin.  The employee sought medical attention and the piece of metal was removed and the wound closed with sutures.  The employee has been placed on restricted duty until the sutures are removed.
