Narrative

While unloading hi-line from a car 10/11/2010 caught the two small toes of his right foot under a wheel. He continued working not seeking medical attention until 10/13/2010 after swelling and pain persisted. X-rays did not reveal any fractures and he was released to unrestricted duty. Became a Restricted Duty after the site of initial injury became infected 10/27/2010.
