Narrative

Emp. was working in #1 Shaft scaling. He was 708'deep. He was struck on the left shoulder with a piece of re-bar. The re-bar was 2 1/2"" long by 1"". The Constructors crew was cutting the re-bar and a piece that was being cut flew down the shaft and hit emp.
