Citation No. 8497782 has been issued on November 1, 2010 for failing to complete a 7000-1 form for an alleged roof fall in the 1st Northwest Panel, #3 entry at spad No. 14+40. River View is submitting this 7000-1 under protest to abate the citation and we do not believe the alleged roof fall was reportable.
