Narrative

Injured was tightening a bolt on the axle of a 9 ton mantrip when he felt pain in his RT shoulder. Having an MRI the injured was found to have a rotator cuff tear and a ruptured bicep tendon of the RT arm. Injured work until surgery on 11/3/10. Injury is being reported as an occupational illness due to repeated trauma over 37 years of working in coal mines
