Narrative

EE was moving charger with fork lift when a fork fell off. He said he tried to replace the fork and dropped it on his foot. He continued to work until 11/8/10 when doctor excused him from work until he gets an MRI. The forks were checked by electrician and he found the forks locked in place and they could not fall off. New forks had been recently installed.
