Narrative

Employee, a roof bolter and has been suffering with chronic pain in his left shoulder for a number of years. Job requires the repetitive motion of bending and installing 6' roof bolts. Employee told supervisor of the pain, but finished his shift. Saw Dr. on 11/22/10 and was given restricted duty. The physician describes his illness as chronic repetitive motion trauma.
