## Narrative

While operating a Continuous Mining Machine on MMU 001-0 section in #8 right x-cut (punch thru), EE was struck on top of his shoulders with a piece of mine roof that appeared to be secured to the mine roof, became loose while mining #8 x-cut into #9 entry. The piece of rock fell from between permanent roof support and the coal rib.
