## Narrative

While driving out liner bolts the bit of the jackhammer became wedged in the hole opening causing the hammer to pull in towards the shell of the mill.  This action caused the employee's hand to become pinched between a mill rail and the trigger of the hammer. This caused severe pinching of all four fingers on left hand.
