Narrative

Employee was removing the discharge hose from a conveyor sump. The hose was hooked to the crane. When the hose broke free from the pump it struck him in the right side of the head. He fell to the ground and was unconscious for a brief time. Was evaluated by a physician and returned to work on 12/2/10.
