## Narrative

Employee was replacing the bit on the Ingersoll-Rand T4 drill. He was using his left hand to steady the hammer while he reached inside the cab with his right hand to operate the control to lower the hammer onto the bit. As the hammer was being lowered, it moved downward suddenly pinching the employee's left thumb and index finger between the hammer and the bit.
