Narrative

Employee was kicking footwall to clean area for setting a timber and sprained his left foot. EE started losing time again on 1/14/11. After further evaluation, it was determined that he has a new fracture that was determined to be caused by the accident on 12/2/2010 which resulted in our reporting him back off work.
