## Narrative

Maintenance employee was trying to un-clog a product line beneath Dust Collector/Rotary Valve. Employee accessed 6"" cap beneath rotary valve and stuck his hand into line and into rotary valve. Rotary valve was not properly locked/tagged out. Injury resulted in partial amputation of three fingers. Employee was transported/treated at local ER, and released.
