## Narrative

Member stated he used forklift to try and pick up a piece of belt and turned sharp, causing the forklift to pivot up in the middle and slam his head into the top. He felt a pop in his neck on the left side. The area was 48"" high and 18 1/2 feet wide. Member was taken by ambulance to the hospital where he was diagnosed with a fractured vertebrae.
