## Narrative

EE was raising a shield at #11. She pulled a staple from a 3/8 hose that was still pressurized. The plug broke out of the hose striking her glasses causing a laceration to her left eyebrow. She was wearing all her PPE. Both foreman were not aware that she was on the section and was doubling back.
