## Narrative

The injured was at 114 shield pulling in 115 shield, he heard rock fall between shields behind him. When he turned, the pontoon from 115 shield caught his right foot. It pushed on the side of his boot smashing the meta-tarsal to the top of the foot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner profiency was a factor.
