Narrative

Employee was helping roof bolt crew to bolt #7 entry, when a rib rolled and struck him. He was standing on the off side of the roof bolter between roof bolter and rib. The rock that struck him was approximately 6 ft. long x 23 inches wide x 11 - 18 inches thick.
