Narrative

Occ. Illness: Employee reported on 1/14/2010 that he has had chronic pain in his right shoulder due to years of mining. No specific incident. Over 1 year later, On Feb. 14, 2011, he finally had surgery on his right shoulder. Feb 10, 2011 was first shift missed, as he had his pre-op appt. on that day. This is an occ. illness.
