## Narrative

A piece of coal struck employee in ankle. Medical was sought,  no fracture, told ankle would get better, full release. Ankle not getting better, appointment was scheduled with orthopedic DR., before appointment there was minor re-aggravation. Feb 10, 2011 he attended scheduled appointment, became LTA with soft tissue injury. Late reporting due to medical records gathering.
