Narrative

EE was checking oil on haul truck. Dipstick is hard to pull due to vacuum effect. She slipped when it broke free and fell on right hand/shoulder. Seen by a Dr. and x-rays showed no fractures. She got a full duty work release and returned to work that day. She continued to experience pain and saw a specialist. MIR revealed torn rotator cuff, she had surgery on 2/23/11.
