Employee placed a 12' extension ladder on a 8"" water pipe to remove a camera. The ladder was not extended and was tied off to the water pipe. After removal of the camera he was descending when the top of the ladder shifted. He lost his balance and fell approximately 6' to 7' landing on a concrete slab, injuring his left ankle.
