Narrative

While hoisting an accelerator jar, the chain at the end of the hoist cable slipped and allowed the accelerator to fall. The bottom end of the accelerator was protruding below the work deck. As it fell the bottom end was elevated and struck the ee on the right shoulder. The ee sustained a contusion to the right shoulder.
