Narrative

EE stepped up and out of a mobile bridge carrier and felt a pain in his lower back. Started missing work 3/14/11. Dr diagnosed as a reoccurrance of an old injury that occurred on 8/17/02 and employee lost time then too. Changed to 8/17/02 - This is additional days off to injury
