Narrative

Injured was using come-a-longs on 3/23/11 and heard his shoulder pop. He kept working & came back to work the next day. He was throwing cable over his shoulder on 3/24/11 and it popped again. This is a re-injury to his shoulder from a previous injury that happened on 8/17/09 where he didn't start missing work until 12/2/09. He had surgery and was able to return to work on 6/15/10.
