Narrative

Employee had completed drilling his lifter holes and was out cleaning out the hole to install his lifter pipe. A grape fruit size piece of rock fell from an undetermined section of the face and struck the employee on the back just below his shoulder. Face had been visibily inspected and washed prior and during installation of collar pipe.
