Narrative

EE was operating a scoop with a 3/8"" and 1/2"" chain connected to it and the other end of the chain connected to the conveyor chain on a continuous miner. He was pulling in the conveyor chain on the continuous miner when the 3/8"" chain broke and a link from it struck him on the left cheek.
