Narrative

Employee pulled rock from drill hole and felt pain in bicep. Referred to Orthopedic who prescribed surgery for several chronic degenerative conditions involving bicep tendons and rotator cuff. Surgery performed on 3/24. Treating physician indicates employee's shoulder condition was due to chronic degeneration rather than a single traumatic event.
