## Narrative

The injured was using a portable Z drill to roof bolt on the face of the 15 Right Longwall Panel, when he started to drill into the mine roof, a piece of rock fell out and struck the injured on top of the head. The injured sustained a fracture of the C1 vertebrae of his neck.
