## Narrative

EE began losing time from work 4/7/11 due to a condition diag. with an MRI. On 3/29/11 he had an MRI done that diag. impingement to the supraspinatus tendon of his left shoulder. He was taken off work 8 days later for the condition cause by degeneration and tendonitis.
