## Narrative

The employee stated that he had just completed installing a corner roof bolt at 5-6 xc. He had his hand resting on the tray of the bolter, and as he was retracting the boom assembly back in toward the roof bolting machine, the roof bolts on the tray of the roof bolter slid forward, pinching his left index finger between the lip on the tray and the roof bolts.
