Narrative

On 4/14/11,employee stopped by the medics office to say his shoulder has been sore for a long time, due to years of working. This was a report only. No incident/ no accident happened. On 4/18/11, employee was on days off & went to doctor, who gave him some restrictions. When he came back to work on 4/20/11, he handed us the dr.note w/restrictions & we were able to accommodate.
