Narrative

Employee stated that he was climbing up on the Dozer from the track to the cab when he slipped and hit his right hip on a light cover. He did not report this specific incident until 4-28-2011 when he asked to see a doctor. He has restrictions that prevent him from doing his job. Lost Time began on 4-29-11.
