Narrative

Employee was getting out of a stockpile loader and hit his knee on the door frame. We filled this out online on 11/18/2010 and reported on our 4th Quarter report. It wasn't until today that we realized that we had not pushed ""Submit This Form to MSHA"" after completing online. Sorry for delay.
