Narrative

A contractor was injured when a panel on the lagging give away causing an aerated concrete to be dispersed out into the slope. This caused an inundation of concrete coming down the slope, overtaking the injured and shoving him down the slope and slamming him into a piece of mining equipment that was parked down the slope.
