## Narrative

Employee slipped on a set of steps but did not fall. He began to complain with lower back pain. On 12/9/2010 he was seen by a specialist and released back to work with no injury listed. The em ployee did not return to work and sought a second and third opinion and was placed off work by a clinic doctor. Employee did not have contact with employer until late April 2011.
